My Neighborhood Primary Care is a community-centered health home which focuses on treating not just your immediate need, but any underlying factors that contribute to your illness or injury. Our Primary Care services include Family Medicine, Adolescents, ideal for children ages 12+, Post Hospital Transitional Care, and Senior Care.
